1) Certain theories of the origin of hypertrophic pulmonary emphysema are discussed and criticized. Some are quite untenable and should disappear from the textbooks.
2) Two theories remain, both having considerable clinical and experimental evidence of their validity. They are: (a) the theory of compensatory distension; (b) the theory of obstruction.
3) It is argued that the theory of obstruction is really merely a subdivision of the theory of compensatory distension.
4) Some of the outstanding features of hypertrophic pulmonary emphysema are explained on this basis.
