The authors present a case of penumopathy of a subchronic type, which became suddenly acute. The x-ray films, in which an extensive atalectasis of the right upper lobe was seen, suggested several possible diagnoses, among which were bronchogenic carcinoma and tuberculosis. The subsequent development of the acute process eliminated these and pointed clearly to the diagnosis of atalectasis due to bronchial obstruction of an inflammatory origin beginning in a chronic infection of the nasal sinuses.
