Seven patients showing spontaneous pneumothorax associated with metastases have been described. These include four metastatic sarcomas, two of Ewing's tumor, and one of metastatic carcinoma. The metastatic carcinoma is the first instance that we know of in which a metastasis other than sarcoma has been the cause of spontaneous pneumothorax.
While one must concede that spontaneous pneumothorax can occur with any type of pulmonary metastasis, there is a distinct preponderance for sarcoma metastasis to be associated with this condition. The exact mechanism of the pneumothorax formation is not clear, but the weight of evidence suggests that most episodes are due to necrosis of tumor nodules with the production of bronchopleural fistulae.
