The mortality rate from pulmonary embolism has shown little change during the past 50 years. Since 1908 when Trendelenburg proposed pulmonary embolectomy for acute massive pulmonary embolism, few successful operations have been reported, and the procedure was almost abandoned. Recent experience with the successful performance of two pulmonary embolectomies utilizing temporary cardiopulmonary bypass reveals the feasibility of this technic. Adaptation of equipment for emergency bypass is recommended to expand the indications for surgical treatment of acute massive pulmonary embolism.
