Three cases are presented which demonstrate the value of direct pneumomediastinography in differentiating bening anterior mediastinal tumors from intrathoracic aneurysms. Clear roentgenographic separation of abnormal anterior opacities from mediastinal vascular structures enabled excisional surgery to progress without prior aortography and with the security that an aneurysm was unlikely. In each instance a thymoma was found. Pneumomediastinography is a relatively safe procedure which can be performed with simple, readily available materials. Pneumomediastinography is recommended as a helpful diagnostic procedure for the differentiation of some benign mediastinal tumors from thoracic aneurysms.
