Focal Myocardial Necrosis in the Cor Pulmonale of Emphysema

Changes occurring in the myocardium of 29 of 30 patients with emphysema and cor pulmonale are defined. These consisted of early degenerative changes of the moycardial fibers and their subsequent replacement by fibrous tissue. Similar focal necrosis was present in only eight of 30 controls. It is suggested that these changes may be related to hypoxia, hypokalemia, or bacterial toxins. These findings, generally unreported in the current literature, bear further study for the elucidation of the clinical discrepancies in the diagnosis, understanding, and management of cor pulmonale and emphysema.
