Highly specific changes in the linear thoracic paraspinal shadow result from para-aortic hemmorhage due to dissecting aneurysm of the thoracic aorta. Widening and displacement to the left of the linear paraspinal shadow immediately above the diaphragm with obliteration or effacement more superiorly should suggest para-aortic bleeding, especially when no abnormality is demonstrated in contiguous vertebral bodies or discs. In a patient with chest pain, such changes in the paraspinal shadow justify a provisional diagnosis of dissecting aneurysm of the aorta and warrant aortographic investigation.
