Vectorcardiographic observations are presented bearing on the phase of ventricular repolarization in hypertensive patients with "left ventricular strain" electrocardiographic pattern. Accumulated evidence suggests that the altered repolarization in "left ventricular strain" is not a secondary phenomenon, subservient to a change in ventricular depolarization, and may well represent a primary effect of abnormal myocardial metabolism and of variation in transmyocardial pressure gradient.
