The electrocardiograms of 19 adults with necropsy-proved severe isolated valvular aortic stenosis were analyzed with particular reference to P wave changes.
Three had normal P waves and 16 had changes of left atrial enlargement. The most sensitive indicator of left atrial enlargement in those patients was a large negative P deflection in lead V-1.
In cases of known isolated aortic stenosis, changes in the P wave in addition to the ST-T changes are of value in assessing the state of the left ventricle and the severity of obstruction.
