Arthralgia in Pulmonary Tuberculosis during Chemotherapy

Twenty-four patients treated for pulmonary tuberculosis with isoniazid and streptomycin are presented who developed arthralgia between two to six months after initiation of chemotherapy. Complete discontinuation of isoniazid caused regression of symptoms.

Pyridoxine was ineffective in the treatment of this syndrome. An interference with serotonin metabolism by isoniazid seems to be an alternate pathway of its toxicity. Corticosteroid and salicylate offer temporary relief; however, physiotherapy proved most effective in those with shoulder involvement.
