Thymic cyst should be suspected when a sharply defined mass is seen far anteriorly in the mediastinum, particularly in relation to the great vessels and base of the heart. Calcification in the margin, when present, supports the diagnosis, as does a palpable extension in the neck. Treatment is surgical resection.
