Recurrent Mitral Stenosis in the Adult — the Contributory Role of Rheumatic Endocarditis

A 30-year-old man underwent mitral commissurotomy for alleviation of his mitral stenosis. He remained asymptomatic for over two years when recurrent mitral stenosis was demonstrated by the reappearance of typical clinical and hemodynamic abnormalities. At open heart surgery, verrucae were found on the mitral valve leaflets. On pathologic examination, the verrucae were most likely rheumatic in origin. This case demonstrates that rheumatic endocarditis can occur in an adult and can play a role in producing recurrent mitral stenosis.
