A patient with an unusual arteriosclerotic aneurysm of an aberrant right subclavian artery is described. This patient presented with an asymptomatic enlarging mass in the posterior-superior mediastinum. The aneurysm was successfully resected, without evidence of symptoms of distal ischemia of the involved upper extremity or subsequent cerebrovascular insufficiency.
