Acute diffuse pneumonia of asthmatics is a syndrome occurring in asthmatics of any age manifested by fever, cough, purulent sputum, leukocytosis without eosinophilia and diffuse pulmonary infiltration in a miliary or peribronchial pattern. A typical case is described in a 65-year-old asthmatic woman. After intensive therapy consisting of tracheostomy, antibiotics and instillation of a mucolytic agent into the trachea, there was rapid clinical and roentgenographic improvement associated with expulsion of mucus plugs from the trachea. This case suggests that the clinical picture of acute diffuse pneumonia of asthmatics can be caused by bronchiolar mucus plugs with infection occurring distal to the obstructed areas.
