An instance of left-sided hemothorax complicating splenoportography is described in a 59-year-old alcoholic cirrhotic patient with esophageal varices. Trans-diaphragmatic laceration of diaphragmatic-intercostal venous collaterals consequent to portal systemic shunting associated with portal hypertension was felt to be the basis of the unusual complication encountered.
