Three main processes underlie the pathogenesis of cor pulmonale—mechanical overload of the right ventricle, hypoxia and hypercapnia. A number of causes of cor pulmonale has been selected to illustrate the varying role of these three processes and the way in which an understanding of their nature provides principles on which therapy can be based.
