Unilateral hyperlucent lung syndrome in a 32-year-old asymptomatic man is presented with a discussion of diagnostic measures and typical findings in such cases. Proper study of such patients may help to elucidate the pathogenesis of this disease and the pathophysiology of related disorders involving unifunctioning lung. Low serum IgM level in this patient could not be readily explained.
