Two cases of metabolic acidosis, one severe, related to an acute and subacute ingestion of isoniazid (INH), have been presented. Elevated serum lactate was documented in the second case and from indirect evidence, is the most likely cause of the metabolic acidosis in the first. The pharmacology of INH and its toxicology at the intracellular enzymatic level has been discussed. In addition, a case of keratitis secondary to INH, which has not been previously described in literature, is presented.
