Two patients are described in whom radiologically evident cavities developed within areas of pulmonary infarction. Constant size, irregular outline and lack of a clear fluid level may be important diagnostic x-ray signs of a sterile, as opposed to a septic, cavity. This differentiation is important because of the different management indicated in the two types of cavity.
