An unsuspected congenital anomaly of venous return to the heart caused an abnormally long and deviant course of a transvenous pacemaker catheter leading to loss of capture and fatal pacemaker failure. Earlier diagnosis could have prompted use of a longer transvenous catheter or placement of epicardial leads by thoracotomy.
