The first heart sound is initiated by a slow deflection in the phonocardiogram. This early wave follows mitral valve closure and precedes the rapid rise of pressure in the left ventricle. It is also present in cases with atrial fibrillation, mitral insufficiency, or mitral stenosis. This component, called o, is explained as resulting from the early stage of left ventricular contraction.
