Narrowed Splitting of the Second Heart Sound upon Inspiration in Patients with Giant Left Atrium

Pericardial traction with compression of the left atrium during inspiration is held responsible for prolonging left ventricular systole and ejection and producing narrowing of inspiratory splitting of the second heart sound in some patients with giant left atria. Inspiratory augmentation of mid-diastolic murmurs heard in the presence of gross mitral incompetence may be attributed to the phenomenon.
