Histiocytosis X commonly involves the lung and pleura, but rarely produces pleural effusion. Two children with histiocytosis X had roentgenographically demonstrated pleural effusions. The effusions were exudates and were presumably caused directly by the disease process. Histiocytosis X should be considered in the differential diagnosis of pleural effusion, especially when the effusion is associated with diffuse interstitial pulmonary infiltration.
