A patient with pulmonary cryptococcosis and pleural effusion was treated successfully by lobectomy. Pleural effusion is an extremely uncommon manifestation of pulmonary cryptococcosis and the biochemical and cellular nature of this fluid is described. Isolated pulmonary cryptococcosis is rarely associated with serious coexisting disease and amphotericin B therapy is not indicated for this entity if the lesion is successfully resected in an otherwise healthy individual.
