Acute dissecting aneurysm of the ascending aorta is a rare complication of selective coronary arteriography. A case is reported of successful surgical repair of the aneurysm and simultaneous revascularization of the coronary arteries with autogenous vein bypasses. This complication of coronary arteriography may be avoided by gentle and careful insertion of the catheter and by avoiding injection when the position of the catheter is uncertain. The technique of repair is applicable to other clinical problems requiring repair of associated aortic and coronary arterial disease.
