A patient with atrial flutter and variable A-V conduction with a ventricular rate of 165 received a single bolus of lidocaine for multiform anomalous beats; the ventricular rate promptly accelerated to 265, thanks to the development of 1:1 conduction. We discuss and emphasize the danger of administering lidocaine in the presence of atrial tachyarrhythmias with rapid ventricular response.
