Considerations on Surgical Treatment of Peptic Esophageal Stenosis

Arne Malm M.D.1

1 Associate Professor of Thoracic Surgery, University of Lund; Chief, University Department of Thoracic and Cardiovascular Surgery, Malmö Allmänna Sjukhus, Malmö, Sweden

In surgical treatment of peptic esophageal stenosis it is of utmost importance to distinguish between functional and organic stenosis. In functional stenosis treatment should be directed towards restoring the incompetence of the cardia using different reconstructive methods. However, if the condition develops into manifest organic stenosis, there is, as a rule, no other choice but to resect the stenosis. In a series of 15 patients, resection was performed and the continuity of the digestive tract was restored by esophagogastrostomy modified by gastroplication. The postoperative results with regard to reflux tendency are encouraging.
