We have documented a unique instance of the development of the Eisenmenger syndrome 17 years after rupture of the infarcted ventricular septum. Our patient represents the longest reported survivor of a postifarction VSD and the only one known to have reversed the shunt as a result of a striking increase in pulmonary vascular resistance.
