John H. Lecky M.D.1 and Alan J. Ominsky M.D.1

1 Cardiorespiratory Anesthesia Service, Department of Anesthesia, University of Pennsylvania School of Medicine, Philadelphia

This paper describes our present method of handling postoperative respiratory problems at the University of Pennsylvania. The preoperative and intraoperative evaluation and management of patients with potential problems are discussed. A systematic scoring system designed to make postoperative extubation decisions less intuitive is presented, along with some specific guidelines for dealing with postoperative hypoxia and hypercarbia. Lastly, our approach to tracheostomy care following surgery is reviewed.
