The case of a nine-year-old boy with traumatic complete spinal cord transection at C2 who did not feel breathless during apnea is presented. Studies conducted during "diffusion respiration" tended to show that neither chemical stimuli due to high [H+] or PaCO2, nor pulmonary vagal receptor activity were capable of inducing respiratory distress. Complete loss of thoracic motor activity and propioceptive information may explain the absence of the sensation of breathlessness when ventilation was suddenly arrested.
