An unusual case of acute staphylococcic endocarditis is reported. Neither the portal of entry for the organism nor septic pulmonary emboli were found. Severe organic tricuspid incompetence developed, the noteworthy features of which included premature opening of the pulmonary valve, a soft pulmonary closure sound and low QRS voltages. The prognosis is probably good and we favor conservative therapy.
