A 34-year-old woman died four months following mitral valve replacement because of nocardial endocarditis. This represents the first clinical report of nocardial endocarditis after cardiac surgery. Intermittent fever, progressively increasing symptoms and signs of congestive heart failure and skin abscesses were the presenting manifestations. The patient died 25 hours following her hospital admission before starting chemotherapy with sulfadiazine, which is believed to be the drug of choice for Nocardia asteroides.
