Obstruction of a disc mitral valve prosthesis (Beall valve, medium size) was detected clinically and confirmed by phonocardiography two months following its insetion. A very short interval from aortic closure to the opening sound of the prosthetic valve (0.05 sec) suggested a high transmitral gradient which was confirmed at cardisc catheterization. The patient died suddenly awaiting surgery and a large thrombus which obstructed more than 45 percent of the valve orifice was found at autopsy.
