A 44-year-old patient with myotonic dystrophy and cardiac involvement is presented with the unusual feature of persistent atrial flutter for over 34 months. Focal myocardial dysfunction demonstrated by left ventriculography simulated similar findings in ischemic heart disease. Coronary angiography revealed, however, normal coronary arterial tree. This case is reported to point out these two unusual features: (1) the chronic nature of the atrial arrhythmia and (2) the focal myocardial dysfunction.
