Symptoms of congestive heart failure first prompted a man with polymyositis, without prior clinical evidence of cardiac disease, to seek medical attention. He was found to be in a hyperkinetic cardiovascular state refactory to treatment with digitalis, diuretics, and bedrest. The use of propranolol relieved the patient's hyperkinetic and congestive signs; however, the myositis continued to progress. Autopsy findings revealed that the patient's heart was microscopically normal. This case suggests the possibility that this unusual circulatory manifestation may occur as part of the polymyositis syndrome.
