An arteriovenous fistula from the anterior descending coronary artery to the great cardiac vein was detected by coronary cineangiography in a 53-year-old man. The fistula was believed acquired secondary to occlusive arteriosclerotic coronary artery disease. Clinical findings and cardiac catheterization revealed evidence of left ventricular failure, and a surgical procedure was undertaken to close the fistula and re-establish normal hemodynamic parameters.
