Albert J. Miller M.D.1

1 Cardiology Section, Department of Medicine, Northwestern University Medical School, and the Northwestern Memorial Hospital, Chicago

A patient is presented who was treated by Osler for rheumatic pancarditis with pericardial effusion at the Johns Hopkins Hospital in 1894. A 59-year followup is presented, along with scrutiny of diagnostic methods and treatment in 1894 and some conclusions about medical care and recordkeeping considered pertinent to modern times.
