A case of pneumonia occurring in a nine and one-half-year-old boy is presented in which the initial x-ray film of the chest showed a round hyperlucent area suggesting malignancy as one of the diagnostic possibilities. The clinical and laboratory findings indicated an inflammatory process as the most likely etiologic agent. A trial of antibiotic therapy was given before extensive investigation for malignancy was undertaken. The patient responded well to therapy, thus avoiding further needless studies such as bronchoscopy and thoracotomy.
