A case of acute appendicitis with perforation and abscess formation secondary to sarcoidosis is presented in a black man with marked multisystem involvement with sarcoidosis. Histologically, noncaseating granulomatous changes with multinucleated giant cells were seen in all layers of the appendix including the mesentery. A discussion of the present case in light of three previously described cases of probable sarcoidosis of the appendix along with the other intestinal forms of sarcoidosis ensues.
