Tissue Penetration of Amphotericin B in Candida Endocarditis

Treatment of Candida parapsilosis endocarditis with amphotericin B failed to sterilize blood cultures despite serum levels which were twofold greater than the in vitro minimal inhibitory concentration. Valve tissue excised at operation continued to yield positive cultures despite prolonged immersion in high concentrations of amphotericin. These findings suggest poor penetration of amphotericin B into the vegetations of fungal endocarditis.
