The present report documents the absence of left atrial "a" waves in two patients with sinus rhythm and longstanding mitral stenosis. In both cases clearly identified right atrial "a" waves were recorded. Left atrial pressure tracings were obtained by transseptal heart catheterization. Ineffective left atrial contraction due to electromichenical dissociation provided a satisfactory explanation for this phenomenoa.
