Fiberoptic bronchoscopy was performed during mechanical ventilation in ten patients in whom excessive secretions and/or atelectasis were thought to be interfering with alveolar ventilation and oxygenation. Arterial blood gases obtained before, during, and after the procedure demonstrated mean increases in Pao2 and Paco2 of 10.0 and 5.2 mm Hg, respectively, during bronchoscopy examination, thought to be due to the fixed outflow obstruction created by the bronchoscopy in the airway. A mean improvement in Pao2 of 14.3 mm Hg was maintained after the procedure, while Paco2 returned to previous levels.
