An echocardiogram showing simultaneous recording of both semilunar valves was obtained from a woman with a large left pleural effusion. The patient demonstrates that this finding is not diagnostic of transposition of the great vessels as previously reported, and may result from an altered recording angle due to noncardiac disease.
