In addition to its favorable bronchodilatory effects in patients with COPD, theophylline administered orally or intravenously clearly enhances cardiovascular performance. Both right and left ventricular systolic pump function are acutely, subacutely, and chronically improved by this agent. The mechanisms for these favorable actions appear to be twofold, namely; positive inotropic effects of theophylline and reduction in ventricular afterload. Physicians should consider these additional positive effects in patients with COPD when contemplating the administration of theophylline.
