In France, as well as in other countries including Latin America, the excellent objectiveness of your rationale for the approach of end-stage Pulmonary Fibrosis in ARF that require MV. But in the US, where I practice, many religious, cultural, political and economical issues have impact in our medical decisions. And unfortunately the aggresive approach, most of the time not indicated from the ethical and medical point of view, will still obligate physicians to intubate and mechanically ventilate this group of patients, even if they are NOT candidates for transplanatation.
