It's quite common to see asthmatics being prescribed metered dose inhaler without a spacer device. In younger children having bronchial asthma the use of babyhaler along with the inhaler increases the deposition of drug in the lung.

Even in adults I do not prescribe metered dose inhaler without a spacer device.The pMDI to be used with the spacer should be shaken immediately before use and fixed upright in the appropriate aperture in the spacer.

It is essential to be sure that the valves of the spacer are moving correctly- an audible click should be heard with the rigid plastic type and the parents should try to observe the flexible valve movement in those for infants. Priming the spacer with a few doses to coat the inside and reduce static loss of drug before using the spacer for the first time and not washing it quite often.
